Excused AbsencesStudents who miss a day of school must submit an Excused Absence Form to the Attendance Office in order for the absence to be excused from their attendance record. Classroom teachers cannot excuse a student's absence from class. Excused Absence Forms is below. Students who miss 3 or more consecutive days of school must additionally submit a doctor's note in order to have the absences excused. Any questions regarding this policy should be directed to the Attendance Office at (702)-799-0180 x4035 DMV Attendance InformationBeginning January 1, 2015, anyone under the age of 18 who applies for an instruction permit, driver’s license or driver authorization card must provide the DMV with proof that he or she meets the minimum Nevada school attendance requirements. The applicant must submit a Certification of Attendance (DMV-301) Form when applying for an instruction permit. If a student’s permit or license is suspended because of truancy, a new DMV-301 must be submitted at the time of reinstating the permit or license. You can fill out a form request here.
The Certification of Attendance form is also used to suspend or deny the driving privileges of a student who has been declared a habitual truant. PLEASE NOTE: Students are allowed 7 unexcused absences for the whole year and 3.5 unexcused absences per semester before driving privileges are revoked.
